upload picture hack install problems

I am in need of some help with phpbb. I have setup and run a board for a while and now need to make some changes but am stuck.

I am trying to install the upload picture hack. I have followed the instructions and done everything there. When I run the install.php (http://forumurl/install/install_uploadpic.php) file nothing seems to happen & I get redirected to the page where it tells me to delete the install directory. And the upload picture options doesn't work???
I get the uploadpic options in the administration panel.
On the main upload section i get the following error message: General Error -An Error Occurred

DEBUG MODE

SQL Error : 1054 Unknown column 'user_allow_uploadpic' in 'field list'

My guess is this is due to the install_uploadpic.php file not being run, but i have no idea why???
I have php and mysql installed and working, the board works fine. I am using ms windows 2000 server and phpBB version 2.0.21. I am using the latest version of the mod version: 1.3.4
